How mosquitoes survive dengue

A new study from Colorado State University sheds light on how mosquitoes, which transmit deadly viruses such as dengue, can survive the infection that kills humans.

The researchers have found that Aedes mosquitoes avoid becoming ill by triggering an immediate, potent immune response.

However, since their immune system does not eliminate the virus, the mosquitoes are able to pass it on to a new victim.

With the new discovery, the researchers now aim to determine how the virus evades the mosquito’s defence because that may help to fight disease by interrupting the growth of dengue virus within the mosquito before it can be transmitted.

The researchers showed that RNA interference - the mosquito immune response -is initiated immediately after they ingest blood containing dengue virus, but the virus multiplies in the mosquitoes nevertheless.

They said that genetic manipulation of RNA interference could be a significant weapon in stopping dengue virus transmission by Aedes Aegypti.

Even a tiny bit of flab raises heart failure risk, claims study

Even a little bit of extra weight can raise the risk of heart failure, according to a US study published on Monday that calculated the heart hazards of being pudgy but not obese.It comes as little surprise that obesity makes a person much more apt to get heart failure, a deadly condition in which the heart is unable to pump enough blood throughout the body.

But researchers who tracked the health of 21,094 US male doctors for two decades found that even those who were only modestly overweight had a higher risk – and it grew along with the amount of extra weight.

In men who are 5 feet 10 inches tall, for every seven pounds (3.2 kg) of excess body weight, their risk of heart failure rose on average by 11 per cent over the next 20 years, the researchers wrote in the journal Circulation.

The average age of the men at the outset of the so-called Physicians’ Health Study was 53. During the study, 1,109 of them developed heart failure.

Overall, the risk of heart failure increased by 180 per cent in men who met the definition of obesity according to their body mass index (BMI of 30 and higher), and by 49 per cent in men who met the definition of overweight (a BMI of 25 to 30). Heart failure, also known as congestive heart failure, contributes to 300,000 deaths each year in the United States.Conditions such as coronary artery disease and high blood pressure can leave the heart too weak or stiff to fill and pump blood efficiently.

Dr Satish Kenchaiah of Brigham and Women’s Hospital in Boston and colleagues also looked at how physical activity affected heart failure risk.

“The lean and active group had the lowest risk and the obese and inactive group had the highest risk,” Kenchaiah said in a telephone interview.

“As far as vigorous physical activity is concerned, even if somebody said they exercised one to three times per month – which is a very low level of exercise – they had an 18 per cent reduction in the risk of heart failure after accounting for all other established risk factors,” Kenchaiah added.

The benefit of exercise in cutting heart failure risk was seen in lean, overweight and obese men, the researchers found. But regardless of the level of activity, higher body mass index also meant higher heart failure risk.

FDA panel set to weigh new female condom

A new, potentially less expensive version of the female condom faces U.S. regulatory review this week when a Food and Drug Administration advisory panel weighs whether they adequately prevent pregnancy, HIV and other sexually transmitted diseases.

The FC2 Female Condom, made by Female Health Co, is made with a synthetic rubber using a process similar to male condoms that the company says is less labor intensive and should reduce its current cost.

Male condoms, which come in a variety of brands and cost consumers between 50 cents and $2 a piece, are far more widely used than their female counterpart, which costs between $2.80 and $4.

Chicago-based Female Health is seeking FDA approval to market the new version. On Thursday, the agency will seek a recommendation from its panel of outside experts before later making its final decision.

“The whole idea is to increase access,” said Mary Ann Leeper, an adviser and former president of the company.

But FDA regulatory staff questioned whether the company should have conducted specific trials to show how well the FC2 prevents women from contracting diseases or becoming pregnant.

Female Health said it did not conduct such studies because FC2 uses a new material but is otherwise similar to the version already on the U.S. market, the FDA staff said in documents released on Tuesday ahead of the panel meeting.

The company “asserts that such studies are not necessary. This is an important review issue,” the staff wrote.

The company said it also looked at durability of the new material, a synthetic rubber called nitrile. The original condom uses polyurethane.

Both versions are comprised of a sheath with a closed ring on one end that is inserted near the cervix and an open ring on the outer end that stays outside the woman’s body.

Conducting another trial would have taken five more years and cost millions of dollars, Female Health’s Leeper said.

“The design is exactly the same, how you use it is exactly the same … we just don’t believe there is any more information required,” she said.

FDA approval could help boost sales in the United States, which make up just 10 percent of the female condom’s 34.7 million unit sales in 2008, according to Female Health Co.

“We haven’t been able to market the product,” Leeper said. But Female Health is seeking to partner with another company, perhaps a male condom manufacturer or a drugmaker invested in human immunodeficiency virus (HIV) awareness, she said.

Most of its U.S. sales are to aid agencies, including the U.S. Agency for International Development, which Leeper said needs FDA to approve FC2 before it can distribute it abroad.

The bulk of the condom’s use is in other countries, particularly in Africa where public health agencies provide it to help prevent the spread of the HIV virus.

The female version gives women their own option for protection and allows them to insert a condom before intercourse. Most other countries have already adopted the newer version, the company has said.

It also offers other advantages over the male condom, according to the company, including greater protection by covering part of a woman’s outer genitals.

Shares of Female Health Co. were up nearly 5 percent, or 13 cents, at $2.98 on the American Stock Exchange.

Two-drug BP therapy can prevent heart attack, stroke

Doctors are usually advised to begin high blood pressure treatment with a diuretic-based strategy, but now an international study has shown that a different single-pill drug combination could be more effective at preventing cardiovascular events such as heart attacks and strokes.

The findings of the randomised study led by a University of Michigan Health System researcher, has actually challenged the way blood pressure is usually treated worldwide.

The researchers in the ACCOMPLISH study compared two drug combinations and the results turned out to be so significant that the trial was stopped early.

Both single-pill combination drugs helped more than 75 percent of people who had high blood pressure and other cardiovascular risk factors get their blood pressure down.

In fact, more importantly, patients in the study taking the combination that included a calcium channel blocker had 20 percent fewer heart-related events than the patients taking the other combination.

For the study, a total of 11,506 study participants took a single tablet that includes two medications. One group received a tablet containing benazepril, which is a type of drug called an ACE inhibitor, and amlodipine, which belongs to a class of drugs known as calcium channel blockers or CCBs. The other pill combined benazepril and hydrochloro-thiazide, a type of diuretic or “water pill.”

Scientists observed a 20 percent reduction in cardiovascular events with the ACE/CCB combination tablet.

Currently, national guidelines for the treatment of high blood pressure (also called hypertension) call for patients who need medication to start out on a single pill, usually a diuretic, or water pill, and to add other drugs only as needed to bring pressure down.

However, the new results may actually change those guidelines. The new study suggests physicians no longer give diuretics preferred status in treating patients.

“This robust study showed us that switching patients to a single-pill combination meant that twice as many patients got to their blood pressure goal, regardless of previous therapy,” said University of Michigan Health System’s Kenneth Jamerson, M.D., the leader of the study.

He added: “The significant reduction in cardiovascular events we observed in patients will, I hope, show physicians that earlier use of a combination medication, especially with amlodipine, may be in the best interest of patients,” he says.

The study results have shown that just six months of treatment with either drug combination was enough to bring the blood pressure of 73 percent of patients into an acceptable range.

The drop in blood pressure was despite the fact that two-thirds were already taking some other medication before they entered the study.

The study has appeared in the latest edition of New England Journal of Medicine.
